Malabar Care Connect - COVID-19 Vaccine Clinic for Families

Dates of the Clinic -  February 18, 22, 25


The Clinic can vaccinate 10 individuals each day. 

Individuals interested in scheduling will need to call 419-522-6191

When speaking to the scheduler, you will need to let them know that

You wish to get the vaccine at Malabar Care Connect. You will be

placed in an appointment slot and will get notified the day before

If they will need to be rescheduled. The Clinic needs to have 10 people

Scheduled per day to run the clinic. 

Please bring your driver’s license or comparable state ID to your appointment


Eligibility - Malabar Care Connect will vaccinate anyone 65 yrs. Old and older OR

Anyone 18yrs.old or older with qualifying  health conditions. 


Qualifying Conditions:


Cerebral palsy

Spina bifida

Severe congenital heart disease (requiring hospitalization in the past year)

Severe type 1 diabetes

Inherited metabolic disorders including phenylketonuria

Severe neurological disorders including epilepsy, hydrocephaly, and mirocephaly

Severe genetic disorders including Downs syndrome

Fragile X syndrome

Prader-Willi syndrome

Turner syndrome

Muscular dystrophy

Severe lung disease including asthma requiring hospitalization within the past year

Cystic fibrosis 

Sickle cell anemia 

Alpha and beta thalassemia 

Solid organ transplants

*Vaccine recipients must be age 16 or older to be eligible for the Pfizer                   vaccine, and age 18 or older to be eligible for the Moderna vaccine.

*Any individuals with one of the conditions above AND an intellectual or developmental disability who have not yet received the vaccine should contact their local board of developmental disabilities for assistance scheduling an appointment.

They do not have to provide proof of their condition. Please watch closely to ensure the correct Target Group/Population is selected when documenting the administration.

We will NOT be vaccinating school employees - Their school will be partnered with a vaccine provider.  We will NOT be vaccinating anyone from Phase 1A.  Anyone who is in Phase 1A asking to be vaccinated should contact a hospital as these are the only vaccine providers to administer them.
